2014 Indiana Rock Star Start Up Kits - Ukulele |
Indiana State Library - Youth Services
Thank you for your interest in a Rock Star Start Up Ukulele Kit!
Fifteen (15) Ukulele Rock Star Start Up Kits are available to Indiana public libraries (limit of one (1) kits per library system). Applicants must demonstrate their need and describe how the kits will be used to facilitate early literacy in their library.
Applications will be reviewed and scored based on demonstrated need and plans for use. Applicants are also required to submit a video, no longer than 60 seconds, answering the following question: "Why should my library be chosen as a recipient of a Rock Star Startup Kit?" The link to the video should be submitted as part of this application. Applicants may use YouTube or Vimeo to submit their video.
Applicants will be notified of their status in December of 2014 and kit delivery will be arranged at no cost to the library. Please visit the Indiana State Library website for more information on the kits.
Applications must be received by 5:00 PM EST on November 12th, 2014
Please contact Suzanne Walker, Children's Services Consultant, if you have questions about this survey: suwalker@library.in.gov or (317)234-5649.
This project is made possible by a Library Services and Technology Act grant from the U.S. Institute of Museum and Library Services administered by the Indiana State Library.
