If you are a registered Environmental Health Specialist or Wastewater Specialist (in accordance with ORS 700 of the Health Licensing Board), a Professional Engineer (in accordance with ORS 672), a current NAWT inspector, or have a similar license or certification (approved in writing by the DEQ), please enter your qualification and certification number below.
