Designing a service to help children with disabilites participate in adaptive sports and recreation |
What is the purpose of this study?
We would like to develop a service to support participation in adaptive sports and recreation for children with disabilities and their families. To design a service that most closely matches the needs of our community we need to collect information about adaptive sports and recreation, preferences, and demographic information from parents of children with disabilities. Although you will not get personal benefit from taking part in this research study, your responses will assist us in designing a service to help people participate in adaptive sports and recreation in the future.
We hope to receive completed surveys from about 1200 people, so your answers are important to us. Of course, you have a choice about whether or not to complete the survey, but if you do participate you are free to skip any questions or discontinue at any time. This survey will take about 15 minutes to complete. There are no known risks for participating in this study.
Your response to the survey/questionnaire is anonymous which means no names will appear or be used on research documents, or be used in presentations or publications. The research team will not know that any information you provided came from you, nor even whether you participated in the study. We will collect information about your zip code and county of residence.
To complete this survey you must be the parent or legal guardian of a child with a disability ages 3 -17, and live in the state of Wisconsin.
