EOB meeting preferences survey

This survey of EOB members is being conducted primarily to get a sense of everyone's comfort with our meeting format and schedule, but secondarily to explore any willingness to perhaps meet via teleconference periodically on topics of strategic import.

Very happy to work with the consensus of the group, but have been made aware previously that folks had opinions, so thought best to check in on those.

Regardless of meeting format, we would make the record of the meeting fully available to the community.
