What is this survey about?
This is a joint questionnaire from Plymouth City Council (PCC) and Northern, Eastern and Western Devon (NEW) Clinical Commissioning group (CCG) to find out your views on the future of health and social in the city.
In recent years in Plymouth people's feedback has frequently told us that you want services to work in a more joined up way so that people can get the right care, in the right place at the right time.
Because of this feedback, both PCC and NEW Devon CCG have together been looking to see how we can make health and social care services work more closely. Our first step is to base the adult social care and health services in the same place in the city, and we are proposing that PCC's adult social care services work for Plymouth Community Healthcare from April 2015.
This survey aims to find out if you agree with this and if you have any views, comments or suggestions that we should take into account. What you think is important and this survey is only part of the engagement with the public that will take place throughout the process of developing this proposal.
The questionnaire is short and should not take you more than five minutes to complete. Closing date is Friday 28 November. Thank you for taking the time to complete it.
