At the Clinical Nutrition Research Center (CNRC) at the Illinois Institute of Technology, we are conducting multiple nutrition research studies. By completing this survey, we can determine whether you are eligible to screen for one of our current research studies. If you are not eligible to screen for one of our current research studies, we can retain your survey and contact you for future studies you may be eligible for, if you are interested.

Study inclusion criteria, requirements, procedures, schedules and compensation will be different for each individual study.

Question Title

* 1. This survey will be used to determine whether you are eligible to complete an in-person screening visit.
The procedures in this pre-screen survey do not involve any known risks.
You will not be compensated for the survey or screening visit. However, if you are eligible for a study after completing a screening visit, you will be compensated based on your completion the study visits/requirements.

Confidentiality will be maintained to the degree permitted by the technology used. All the information you provide will be used responsibly and will be protected against release to unauthorized persons.
Your participation in this online survey involves risks similar to a person’s everyday use of the Internet.
