Early Intervention Training Program FY15 Needs Assessment

* 1. We are conducting a study to determine the training needs of Early Intervention Providers. This study is part of the work of the Early Intervention Training Program at the University of Illinois. For purposes of this study, we would like to invite you to participate by completing the Needs Assessment Survey. The Needs Assessment Survey can be completed in approximately 5-7 minutes. You will not be required to include your name in the form.

We will be aggregating the data from the completed survey and analyzing them as a group. Upon completion of the project, we will destroy all the individual data collected from this study. Results of this study will be used for a final report due to the Illinois Department of Human Services, journal articles, and conference presentations. In any publication or public presentations related to this study, pseudonyms will be substituted for any identifying information. We want to assure you that information derived from your completed survey will be held in strictest confidence, and that you may withdraw from the study at any time without penalty. Your participation in this project is completely voluntary and your choice to participate or not will not impact your current and future participation in any trainings offered by EITP, your job, and your status in our field. We do not anticipate any risk to this study greater than normal life and we anticipate that this project will contribute to the improvement of training in the area of early intervention.
