Lives Lived Well is one of Queensland’s leading alcohol and drug support organisations, delivering a range of services and programs across the state.
We have recently been funded by the State Government to establish an Alcohol Harm Prevention Service (AHPS). Our aim is to engage with as many Queensland communities as we can to reduce alcohol-related harms.
We need your help!
By completing this survey we can get a better understanding of what's happening in your community. It will also help us to build a tailor-made service that will help connect communities experiencing similar issues and develop a range of resources that local people can use to address local issues.
The more people and groups that we can connect with, the better the information we will have, so please feel free to send this on to others!
The survey takes approximately 10 to 15 minutes to complete. Most questions are multiple choice but some ask for more detail. Really important questions requiring a response are marked with an asterisk (*). The questionnaire will not progress if these questions are not answered.

Your responses will be kept strictly confidential. Only collated data will ever be reported, meaning that individual responses will not be identifiable. If you choose to provide your details at the end of the survey, these will be stored separately from your survey responses and will only be accessed by our team to contact you if you have indicated you would like this.
