Maryland Hospitals for a Healthy Environment (MD H2E) invites you to participate in a survey developed in partnership with the Johns Hopkins Center for a Livable Future’s Maryland Food System Map Project. Together, we are examining the food system in Maryland to better understand how food moves from farm to hospital.

We recommend you have the following purchasing data accessible before you start the survey:
