1. 2014 USA Ultimate High School Westerns Application

Thank you for your interest in attending the 2014 USA Ultimate High School Western Championships. This online form will save time and allow us to process applications in a more efficient manner.

The 2014 Western Championships will be held in Corvallis, OR on May 31-June 1. High school teams in the western region are eligible to apply for High School Westerns.

Applications must be completed by Friday, March 21, 5pm Mountain Standard Time. Notifications will be emailed to invited team (and alternates) by Friday, March 28. While the application can be completed by any member of the team, an adult (coach, chaperone, teacher sponsor) must be the primary contact.

For teams that receive a bid to the tournament a deposit for 10 players ($350) is due by Friday, April 11 (5pm Mountain Standard Time) or an alternate team may be given your spot. This deposit will be applied to the full tournament fee. All remaining materials are due no later than two weeks before the event.
