I plan lung patients. About 60% are SBRT so I use the 6FFF. For the other ones are use 6MV.

We use 6MV for larger lesions and 6fff for smaller, high dose tumors especially SBRT

I will use a 10 MV beam if there is more than 10 cm of soft tissue before that beam hits air in lung.

10MV is the bomb, Clinac 18 had it right in 1976. 10fff is great for SBRT outside lung

We love using 10FFF for lung SBRTs. We do however only bump up our dose rate to 800.

We have matching TrueBeams, in case and machine goes down, we can move patients between machines. But only one of them has 10 FFF so we don’t use it. Because a patient with 10 FFF and the machine goes down cannot be treated.

Interesting how we can get so much more yield at 10fff (2400 MU/min) than 6fff(1400MU/min)
