Unless restricted to the tangent beam angle it generally results in unacceptable low dose to normal tissue. With a little time and effort an acceptable plan can be created with FIF

Typically use IMRT for thin Chest wall irradiation areas where excessive lung doses appear

Cannot seem to obtain insurance approval, therefore we are using FiF, even with SIB plans for L side

We MapCheck our tangent fields and charge a spec phys cons. We feel that this is needed to verify the transfer and accuracy of the unique MLC patterns.
