Your Community Health Services, Your Say!
Nottingham CityCare provides key health services in the community. This includes health visitors, district nurses, home-based nursing, rehabilitation and care for long-term conditions. They also offer a range of children’s public health services and run the NHS Urgent Treatment Centre.
CityCare delivers services across the city in places like health centres, children’s centres, nursing homes, GP practices (although they don’t run GP practices), and in people’s homes.

They are Nottingham’s largest provider of NHS community health services, supporting more than 450,000 patients every year. If you’ve received health care in your local community, it was probably a CityCare service.
CityCare want their services to be easy to access, meet the needs of their patients and families, and provide effective and efficient care. They’ve asked Forever Consulting (social researchers working on behalf of CityCare) to find out how residents would like their NHS community health services to be delivered. This survey only takes 10 minutes, and your answers will help them improve their services.
