PLEASE NOTE
You must complete all enrollment tasks (including registration fee payment and testing) to take classes. If you do not submit the required documents, you will not be given the option to take the required test.

Please photograph / scan and upload. Birth Certificate alternatives include: Passport, Naturalization Certificate, Permanent Residence Card ("green card"), Employment Authorization Card, or immigration Visa. Social Security cards are NOT a valid alternative.
