Important instructions for filling out the Survey:
This survey should take no more than 20 minutes to complete. Please fill out the survey as completely as possible.
Your answers will be used in creating a detailed level benchmarking report tailored specifically to the health and benefit plans offered by tribes for their tribal member populations. Should your tribe choose to participate, then all information provided will be included as part of an overall aggregated benchmarking study. Aggregate results will be published for comparative purposes, but your unique responses will otherwise remain confidential. As a participating entity, the chosen name of your tribe or confederation of tribes will be listed only a participant in the study itself. All tribal entities that complete the survey, will receive a complimentary copy of the report.
Important! This survey set of questions were developed specifically for Tribal Member Plans. The entire survey refers to questions that reference the benefit offerings provided by tribes to their tribal member populations, regardless of employment status. Many health plans may be available to both eligible tribal members and employees of a tribe, such as an enterprise, government/administration, or etc. When answer questions, please limit responses to those that strictly pertain to the tribal member population.
