You need not answer all the questions to complete this survey. By participating, you agree to have all of the information (including name and contact information, if provided) shared with the FDA. In addition, the information may also be used for future research purposes, with your name and any identifying information removed. Your information will be maintained on a secure, password-protected website. There are no anticipated risks in participating in this survey. You may refuse to participate, and you may stop and exit the survey at any time.
