Rural Network Year 2 Evaluation

Volunteer Survey

Survey results will be delivered to the Steering Committee and used for annual evaluation.

This anonymous survey helps gauge our strengths and where we can improve. Please be candid, and keep in mind it is OK (and even expected) that this network may not be strong in all areas.

For support with this survey, please email preventcoalition@esd112.org.
