Thank you for supporting our survey. We are Canada’s Pediatric Palliative Care Alliance, a Health Canada funded initiative with the goal of improving equitable access to high quality pediatric palliative care (PPC) for all Canadian children and families. To reach this goal, we need to hear from families who are caring for, or have cared for, a child with a life-limiting illness (including pediatric palliative care, perinatal palliative care and/or bereavement services).
Families who care for, or who have cared for, a child with a life-limiting illness know so much about the health care system. Understanding your experiences will help us leverage system-wide changes by better understanding what we do well and supporting growth where it may be needed. Please help us meet our goal by taking 5 minutes to fill out this survey sharing your views on the current state, needs, and strengths of pediatric palliative care in Canada.

Your decision to participate in this survey is entirely voluntary and will not affect your relationship with the project team or partners. Whether or not you choose to provide any identifying personal information (e.g., anecdotes or an email address), the information you share will be kept strictly confidential.

The information you provide will be used to help improve pediatric palliative care in Canada. No identifying information will be shared with the public, and all data collected will be stored securely. No scientific publications will result from this survey. Any conference presentations or reports to the funder (Health Canada) will strictly respect your privacy and confidentiality.

Research ethics board approval was deemed unnecessary for this survey due to its focus on informing the Alliance rather than research publication. If you have questions about how the information you provide will be used, please do not hesitate to contact a member of the project team.
