Dementia Practice Survey for Patient Navigation Assistance

The following survey was designed by the New IDEAS Study Team to answer common questions from patients who are interested in participating in the New IDEAS Study. Your responses will help future New IDEAS patients understand what to expect when they visit your practice and simplify the process for accessing healthcare. 

Additionally, your responses will help the New IDEAS Study Champions bridge the gap in knowledge between patients and practices and enhance our community engagement approach in our community-engaged metropolitan regions across the country.
