This Permission and Release applies to the Library of Congress National Library Service for the Blind and Print Disabled (NLS) My Talking Books on Smart Speaker Open Beta (hereafter “Open Beta”) and any materials provided in connection with the Open Beta.
By signing below, you warrant that you are the parent or legal guardian of the minor child whose name is written below. You agree to allow your child to (1) participate in the Open Beta and (2) respond to surveys related to the Open Beta.
Registration in the Open Beta is optional. At the time of Open Beta registration, NLS will be gathering a limited amount of personally-identifiable information (PII) on the child joining the Open Beta, including their full name, email address, phone number, and age. Your electronic signature constitutes acknowledgment to gather this PII.
The PII collected will be used to identify individuals within the Open Beta and to ensure accurate contact information is available for NLS to (1) process your child's Open Beta registration and (2) contact the Open Beta member for the purpose of conducting surveys. Your signature constitutes acknowledgment for NLS to (1) process the Open Beta registration and (2) contact the Open Beta member for the purpose of online surveys to share their user experience during the course of the Open Beta. You are permitted to answer surveys on behalf of the Open Beta member. No additional PII will be gathered. However, responses are not anonymous and will be associated with the child’s BARD email address.
Survey results will be used to understand successes and lessons learned from the Open Beta as well as opportunities to improve the patron experience. No PII will be disclosed by NLS or third-party survey vendor(s). Reasonable procedures will be utilized to protect the confidentiality, security, and integrity of PII collected from children. NLS may reproduce, display, or otherwise make use of anonymized (i.e., not personally identifiable) survey results.
