Consent to Participate in Research

Title of Project: Active Duty Military Women and Veterans and the experience of Maternal-Infant Attachment
Request to Participate in Research:I invite you to participate in a web-based online survey. The survey is part of a research study whose purpose is to better understand Maternal-Infant Attachment among military women and Veterans.
We anticipate about 150 participants for this study. This survey should take about 10 minutes to complete. I am asking you to complete the survey in this study because your participation will contribute to the current body of knowledge regarding Post-Traumatic Stress Disorder (PTSD) and Maternal-Infant Attachment.
The inclusion criteria for the study sample are: You are between the ages of 18 and 45, are a active duty military women or Veteran that has served in a current conflict war; Operation Iraqi Freedom and Operation Enduring Freedom and have given birth 3 to 6 months ago and have a diagnosis of PTSD.
The exclusion criteria are: You are not between the ages 18 and 45, an active duty military women or Veteran that has served in a current conflict war; Operation Iraqi Freedom and Operation Enduring Freedom and have not given birth in the past 3 to 6 months ago and do not have a diagnosis of PTSD.
The decision to participate in this research project is voluntary. You do not have to participate and you can refuse to answer any question. Even if you begin the web-based online survey, you can stop at any time. There are no foreseeable risks or discomforts to you for taking part in this study. There are no direct benefits to you from participating in this study. However, your responses may help us learn more about Post-Traumatic Stress Disorder and Maternal-Infant Attachment.
Your part in this study is anonymous to the researcher(s). However, because of the nature of web-based surveys, it is possible that respondents could be identified by the IP address or other electronic record associated with the response. Neither the researcher nor anyone involved with this survey will be capturing those data. We are making every effort to ensure that no one knows what your responses were on the survey. Survey Monkey is a well known company that collects data for online survey research. The researchers have purchased an encrypted version of their product to reduce the risk to subjects that their responses will be viewed by unauthorized persons. However, the study is not being run from a secure http server such as those used to handle credit card transactions, so there is a small possibility that responses could be viewed by unauthorized third parties, such as computer hackers. Any reports or publications based on this research will use only group data and will not identify you or any individual as being affiliated with this project. The data resulting from this study will be stored for a minimum of three years.
