I have attached to this application three (3) high quality images of my products.

I have attached Public Liability Insurance certificate or am willing to obtain public liability for the event and submit one (1) month before event.

I give permission for Hazelhurst Arts Centre to use these images for promotional purposes.

If selected as a stallholder I will ensure that the visual merchandising of my work to be of a high standard.

I agree to notify Made by Hand organiser one (1) month prior to the event if I can no longer attend. No refunds are issued for cancellations within one (1) month of the event.

Outdoor stalls will be cancelled with full refund in the event of extreme or prolonged wet weather with respect to the safety of our stall holders, visitors and staff as determined by Hazelhurst Arts Centre.

All applicants will be notified by email and advised of outcome after the application closing date. Successful stallholders will be emailed invoices for their stall and payments must be made by the due date.

Please ensure that the terms and conditions attached to the invoice are strictly adhered to as failure to pay by the due date may result in forfeiture of the stall.
