The Mississippi Valley Workforce Development Board, as part of the Job Quality Academy, is conducting research in this region to understand in what ways employers recognize and integrate the "Good Jobs Principles" within their organizations. You play a pivotal role when it comes to the good jobs principles within your organization and the Mississippi Valley Workforce Board has identified you as a company representative to complete this short survey. Your time and input are very important to us. The survey should take less than 15 minutes to complete. Your responses will not be connected to you individually or to your organization, as your responses will remain anonymous. Thank you in advance for your time.
Informed Consent for Job Quality Academy Research
Title of Project: Understanding the Good Jobs Principles from the perspective of employers and employees/job seekers throughout the Mississippi Valley Region.

WHAT IS THE PURPOSE OF THIS STUDY?
This study is being conducted as part of a national Job Quality Initiative and our goal is to collect data from employers, employees, and job seekers to better understand the good job principles in our region.
HOW MANY PEOPLE WILL PARTICIPATE?
We expect that employees and job seekers in a variety of job industries will participate in the survey throughout the Mississippi Valley Region. We expect to have at least 500 surveys from employees and job seekers and 250 from employer representatives.
HOW LONG WILL YOU BE IN THE STUDY?
We are asking that participants complete only one 15 minute survey.
WHAT ARE THE RISKS AND BENEFITS OF THIS STUDY?
We do not anticipate any risks by completing the survey. The survey is anonymous and responses will not be connected to anyone personally or to any company. Participants may not directly benefit from completing the survey, but the input that participants provide will help us to understand what employees, job seekers, and employers consider as part of a good job. We believe that the benefit of knowing this information outweighs the risk of sharing your thoughts in the survey with us.
HOW WILL MY INFORMATION BE KEPT ANONYMOUS?
There will be no personal information collected about the participants in the survey. After the data are collected, downloaded, and analyzed data will only be reported in aggregate form.
WILL I BE PAID FOR PARTICIPATING IN THIS STUDY?
We are offering employees and job seekers who complete the survey an opportunity to be in a drawing for one of five $100 gift cards. At the end of the survey you can add your phone number to be entered into the drawing. After the gift card winners are determined and contacted, the phone numbers will be erased from the data.
WHAT ARE MY RIGHTS IF I COMPLETE A SURVEY?
Taking part in this research and completing an online survey is completely voluntary. By completing the online survey you consent to participate.
