2021 Summer Grief Resource Center Interactive Workshop & Support Group Registration

Thank you for your interest in Horizon Home Care & Hospice's Grief Resource Center programs. These (free) interactive workshops and short-term support groups aim to bring new practices into our experience, while helping to build a community during times of grief.

PLEASE NOTE: Due to the interactive nature of these programs, there is a multi-step registration process.

1) Fill out this form.

2) You will receive an email from Grief Resource Center staff with 3 additional forms that will need to be completed through DocuSign (consent for services, virtual consent, and a client information form).

3) When those forms have been returned, you will be fully registered and will receive the meeting information.
