Informed Consent Form
Quantitative Study Integrating the E-commerce Acceptance Model with Conversion Rate in the Education Market
Purpose.
A research study is being conducted for a doctoral dissertation at Northcentral University in Prescott Valley, Arizona, and you are invited to participate. The purpose of this study is examine how consumer perceptions of educational products websites relate to purchase behavior on the website. You will be asked to explore an educational products website and to answer a series of questions related to your opinion of the website.
Participation requirements.
You will be asked to complete a series of questions related to your beliefs about a particular website and the associated company. Additionally, you will be asked to share information related to your gender, age, educational level, household income, educational role, location (country), and customer status. This information will be submitted via an online survey.

Potential risk/discomfort.
There are no known risks for participants of this study. Personal information you share will include demographic information and attitudes towards a company’s website. You may skip any question you prefer not to answer and may withdraw from the study at any time by exiting the online survey.
Potential benefit.
The proposed study will extend the e-commerce acceptance model by creating the e-commerce website performance model. Society will benefit from the proposed study. First, the study will address the gap in the academic research related to conversion rates. Second, it will provide a way to use e-commerce acceptance model theory to describe e-commerce website effectiveness. Third, it will help online business owners identify factors that contribute to success in the e-commerce environment. The benefits of the proposed research outweigh the risks which are minimal.
Anonymity/confidentiality.
All data collected in this study are confidential. You will not be asked to provide your name or other personal information that would enable your survey responses to be linked to you. You will be able to download the complimentary workbooks without disclosing any personal information.
Right to withdraw.
You may withdraw from the study at any time by exiting the online survey. You may also skip survey questions you don’t feel comfortable answering.
