You have been invited to participate in an online survey about cancer survivorship needs. Georgia CORE will summarize the information you provide to help cancer centers and professionals in Georgia create programs that address your concerns.

Data from the survey may help develop education, services, and/ or programs for cancer survivors in Georgia.
This survey asks questions about (1) your treatment experience and overall health status; (2) availability and access to support services; and (3) whether these services are meeting your unique needs.
Results will address gaps in survivorship care and improve services and resources available to the survivor community.
