Please complete the SAPC Monthly Required Data Report by the 10th of each month using the prior month's complete data.  For the questions below, SAPC requests encounter level data, not unique-individual level data. 
 
Please use whole numbers only for numerical data requested, including percentages.  Do not include fractions, decimals, symbols, or text in numerical-data responses.  For questions requesting percentages, percent-based responses should add up to 100% unless otherwise noted.

Since a Client encounter may involve more than one type of service or linkage, the sum of numbers below does not need to match the total number of encounters in question 5 above. 

For each type of training listed below, there are two consecutive questions.  For the 1st question, give the number of trainings given, including both 1-on-1 and group trainings.  For the 2nd question, give the number of Clients who participated in or received that type of training. The same Client who participated in two trainings would be counted separately for each training.
