This is the official nomination form for the 2022 CNBC Disruptor 50 List. Please review the following instructions and eligibility requirements carefully before filling out the form.
Please submit this form by 3pm ET Friday February 11, 2022

ELIGIBLE NOMINATORS: Nominations for the CNBC Disruptor 50 may be submitted by employees of the nominee company, investors in the nominee company, or any of their communications representatives. ONE ENTRY PER COMPANY, PLEASE. It is not necessary to submit more than one entry per nominated company.
COST: There is no cost to nominate a company for the Disruptor 50 List.
ELIGIBILITY REQUIREMENTS: To be eligible for the 2022 Disruptor 50 List, companies must be:
- Privately held**
- Independently owned
- Founded no earlier than January 1, 2007
**"Privately held" means: The company is not listed for trade on any public stock exchange, and is not in the process of merging with a special purpose acquisition company (SPAC). Any company that has announced, but not yet closed, a merger deal with a SPAC is considered PUBLIC and not eligible for the Disruptor 50 List.
Nominees that do not meet these eligibility requirements will be disqualified from consideration. Nominees that are found to be publicly traded, owned by another company, or entered into a merger agreement with a SPAC will be disqualified from consideration. Nominees that go public, are acquired by another company, or enter into an agreement with a SPAC before April 1, 2022 will also be disqualified.
OFF THE RECORD RESPONSES: Responses to some questions in section 5 of this form will be kept OFF THE RECORD, and used solely for the purpose of scoring. Withholding this information will not disqualify the nominee company from making the list, but could place it at a disadvantage to companies that provide more complete responses.
