The purpose of this survey is to:

-obtain additional data and input from families to be used in planning for reopening of CMS (PK-12) this fall.

Please respond to this survey Once for each individual child in your household who will be enrolled in Pre K-grade 12 in the upcoming school year.

(2) Hybrid Model where students attend school in face-to-face format 2 days per week supplemented with 3 days of remote learning.
