Diabetic Peripheral Neuropathic Pain Pre-screening Questionnaire

Apex Trials is conducting a double-blind, randomized, multiple dose study to assess the efficacy and safety of a medical Cannabis aerosol via an inhaler as an add-on treatment of diabetic peripheral neuropathic pain.

The study will involve a total of 15 visits to our clinic (located in Guelph, Ontario) and 6 remote phone calls over a 22-week period. Reimbursement of up to $1000. Amount prorated based on visit completion.

Please provide complete and honest answers to the questions to ensure this study is a good fit for you. All information collected will be kept confidential and will not be shared without your permission except as required by law. The questionnaire will take approximately 3 to 8 minutes to complete. Your answers will be recorded, but your information will only be used for the purposes of this study.
