I have read the Student Participant Information Sheet, or someone has read it to me in a language that I understand, and I fully understand the purposes, procedures, and risks of the pilot as described in the Student Participant Information Sheet.

I have had an opportunity to ask questions via the aitutor@learningnetwork.com.au email, and I elect to have my child participate in the pilot being completed by High Resolves Group. I understand that I am free to withdraw my child at any time without affecting my relationship with the researchers.
