Sunday Friends New Membership Application

Thank you for your interest in becoming a member of Sunday Friends!

Please note that we will ask you to upload a picture of an ID for each member of your household so please take pictures of all the IDs before you start this application.

For adults, the ID should include the name, date of birth, and address.

For children 17 years and under, we accept a photo of any document that has their name and date of birth.

Please know that your progress will only be saved if you use the same link and device that you started the application with, and responses are saved after pressing the NEXT button at the bottom of a page.
