Community Partners Engagement Survey

Comagine Health is partnering with the Washington State Department of Health and ten other community organizations to implement the COVID-19 Community-Driven Outreach program. This program is designed to improve COVID-19 and Influenza vaccination access and equity and related social determinants of health in the Black or African American community.

Through Community Health Worker (CHW) led outreach, community events, and broader community engagement efforts, we seek to:

Assist community organizations in developing meaningful, culturally acceptable, and accessible communications
Grow existing infrastructure and CHW workforce capacity
Increase screening and follow-up on immunizations
Improve connection to resources for people with chronic conditions and other related health and social needs

These activities serve to strengthen the relationship between communities and public health by building trust and expanding access to health information and services. See the program flyer for more details.

A combination of CHW led outreach and community engagement activities will be used to involve community members to identify equity gaps and increase awareness and engagement in positive health behaviors.

Which of the following ways would your organization like to be involved (check all that apply):

My organization would like to be added to an email list to receive updates and relevant information about the project engagements. We can use this information and distribute it to our social media pages, organization listservs or handouts.

My organization would like to participate in surveys and discussion forums. In these surveys, my organization can provide feedback on the usability and accessibility of the educational materials and engagement tools.

My organization would like to participate in quarterly meetings convened by Comagine Health. These meetings will provide an opportunity for community health workers to discuss their progress and barriers as a cohort.

My organization would like to participate in a quarterly Community Advisory Committee convened by Comagine Health and the Washington Department of Health. There will be a total of 9-12 representatives selected across Washington state to:
