Development of an Online Transition
Program for Students with Mental Health Disabilities going into Post-Secondary Institutions in Ontario

Thank you for your interest in participating in a survey to help us develop a transition program for students with mental health difficulties who are going into postsecondary education. Your participation will involve completing a 26 item on-line survey. Please be assured that the survey responses will be completely anonymous. It will not be possible for individuals to be linked to the answers that they provide. For the purposes of the evaluation no individual responses will be utilized; rather, only aggregate responses will be used.
