Carbal Medical Services employs trained Aboriginal Health Workers to provide Hearing Assessments for Aboriginal and/or Torres Strait Islander Prep to Year 3 students who attend Public Primary Schools. Carbal also provides hearing assessments for pre-school aged children aged 4 and up because our goal is prevention and early intervention.
The following is an electronic permission form that submits your details confidentially to the Carbal Indigenous Child Health Worker. Alternatively you can download the paper form and provide a copy to your school.
