Household Information Survey

Even though free, healthy, and nutritious breakfasts and lunches are available to all students ages 18 and under this school year, we still need each Traverse City Area Public Schools (TCAPS) family to submit an education benefit form. This information is important because it is used to determine the distribution of other educational funding that supports TCAPS students.

For each student in the household Pre-K through 12th grade, list the last name, first name, birthdate, grade level, school, and H if Homeless, M if Migrant, R if Runaway or F if a Foster child.
