Family Resources Survey

The purpose of this survey is to gather information from family members and those caring for children with disabilities about resources and information they have found useful in learning about their child’s developmental and special education services. All information provided in completing the survey is confidential and will not be linked back to you.

The Ohio Inclusion State Leadership Team intends to use the information to create a statewide online resource guide for parents/caregivers and professionals.
