This form collects your name and cell phone number for transmittal to 42Chat. They will contact you via text before your meeting to request that you upload a photo of your proof of vaccination (including booster) or a NEGATIVE Covid test (antigen or PCR).

It may take up to 48 hours to move your information from this form to 42Chat so please register as soon as possible before the event.

Your information will NOT be shared with any other individual or organization. However, you must re-enter your information for each ASEE meeting you will attend.
