Yakima Valley Memorial  Community Health Needs Assessment Survey

As part of Yakima Valley Memorial’s commitment to creating a healthier community, we are inviting you to provide input into the 2022-2025 Community Health Needs Assessment (CHNA). The CHNA process requires an evaluation of the health needs of a community, including a health profile of the community's population, its health status, and socioeconomic outcomes. It also requires gathering input from the community regarding unmet health needs to better identify the most central community needs to improve the health and wellbeing of our community.

Your insights and feedback regarding our community's healthcare needs and gaps will be invaluable to this process.
