Thank you for being so interested in helping us understand your experience of care provided after being discharged from hospital.
Survey information
We'd like to hear your views if you, or someone you care for, has been discharged from hospital between January 2023 and March 2024 to any of the following places:
- Discharged home with support from 'Home First' or 'Reablement'*
- Moorland Grange in Huddersfield
- Ings Grove in Mirfield
- Brackenbed View (Oak Unit) in Halifax
*Home First or Reablement is a short-term service which supports people to maximise and maintain independence. It helps people to retain or regain their skills and confidence so they can learn to manage again after a period of illness
This survey is for people over the age of 18 who live in Kirklees or Calderdale
