Thank you for taking the time to complete the Texas Library Association's survey on libraries' use of existing HB 2128 telecommunications discounts for broadband services.

The purpose of this survey is to gather concrete information as to the dollar value of this discount program and to ascertain the number of libraries benefitting from the discounts.

The questionnaire gathers information about the state's telecommunications discount program which was orginally created in 1995 as part of the Public Utility Regulatory Act. The discount program was then reauthorized in 2007, and some discounts are set to expire on Janury 1, 2012.

This survey attempts to gather LIBRARY broadband information at the system wide level (i.e., for a whole school district, for the whole academic institution, or for the whole public library system--not individual campuses or branches).

The survey should be completed by someone with access to system-wide information regarding costs, telecommunications issues, and the discount program. Even if your library does not make use of the HB 2128 rates, please complete the survey questions where possible. It is helpful to know that broadband services libraries are using and what is the cost for those services. Information about the extent of the use of broadband services and the state discount program is extremely valuable.

A. For population served: if an academic library, count students, faculty, and staff; if a public library, list legal service population number; and if a school library, count students, faculty, and staff.
B. For physical library outlets: if a public library system, count main and branches; if an academic library, count all campus libraries; and if a school district, count all campus libraries within the district.
