The following survey is being used as part of an evaluation of the YOU(th) Decide program, NOT an evaluation of individual youth.
We are interested in how lots of students respond to these questions. We are not especially interested in any one person’s answers. Your name does not appear anywhere on the survey, so your answers are confidential. No one will ever know how you answered these questions – not your teachers, parents, friends, or other students.
The survey should take you less than 5 minutes to complete. The questions included in this survey are about the myths and facts of gambling and media literacy, as well as feedback on the design of the program. This survey is NOT a test.
