COVID/Long COVID Survey

The following is a short research survey about your experiences with COVID-19. Please complete the following survey if you have tested positive for COVID-19 or have had a positive COVID-19 antibody test. This survey will ask you questions about your medical history, COVID-19 symptoms, and recovery. Your participation in this research survey is voluntary and individual questions are optional. If you complete and submit the survey, you consent to participating in this study.

This research survey is being conducted by Dr. Christine Sang and the Translational Pain Research Program at Brigham and Women’s Hospital in Boston, an affiliate of Harvard Medical School.

We are asking you to participate in this research because your answers to this survey will help us better understand the experiences of those suffering with Long COVID. Please be assured that your responses are secure and confidential. Your personal information will never be shared with parties. Your individual responses will not be shared, but grouped results may be shared.

If you are under the age of 18, you are not eligible to participate in this research survey. However, we are still here for you!
