After you have submitted the form, you will be directed to a link to download an EXCEL spreadsheet that you will use to document your field experience activities. You will need to copy and paste the URL link into a new browser window to download the file.  Students are responsible for recording their experiences weekly and sharing their logs with their site and university supervisors.  If you have any questions, please contact your course instructor for assistance.
