Town of Blackstone Stormwater Information Survey

The Town of Blackstone is seeking information from residents and businesses with respect to their understanding stormwater related issues in the community. In order to better understand what the Town must do to inform residents and businesses, we ask that you complete the following short questionnaire and return it to us either electronically or in hard copy. Hard copies may be hand delivered or mailed to the Department of Public Works 53 Elm St. Blackstone, MA 01504.
