Thank you for your interest in the Lead Hazard Reduction Grant Program. This grant helps eligible homeowners and renters by funding the renovation work needed to eliminate or control lead paint hazards in their homes.

The Evanston Health and Human Services Department is assisting the Cook County Department of Public Health with determining applicant eligibility. 

You will be contacted within 2 weeks to discuss the results of the review and to determine next steps.


Before proceeding, please have a legible copy of the following documents (not all may be required):

- Income Information - For anyone living in the home who is 18 and older
        - Last tax return including schedules, attachments, W-2's, AND
        - Last two check stubs  
        * If you don't have a tax return or check stubs, a letter from your employer stating your income.

- Most recent documents showing any other income source (example: rental income, IRS 1099)

- If receiving social security or other benefits, submit most recent benefits statement

- ONLY if not working and aged 18 or older: a signed and notarized No Income Verification form is required.

- ONLY if child visits but does not live with you: a Significant Time form must be completed.

- Property Deed

-Current real estate tax bill
