  • This form is to add an individual whose employment involves working out on the ocean in any one of several US commercial offshore industries to our waitlist for a free PLB-Personal Locator Beacon, assuming all required information is provided and meets review.
  • DO NOT submit a request for a free PLB from us if you and your family can pull together money to buy one. Do the right thing and order one online as soon as possible from ACR Artex or McMurdo.
  • DO NOT submit a request for a free PLB from us if your company already provides a PLB for employees to use.
  • Sending you a free PLB depends on how many donations we have received from others. No donations, no PLBs.
  • Expect to wait over a year to receive a free PLB, if you cannot afford one and your request is accepted.
  • YOUR REQUEST WILL BE DENIED if one of these specific official U.S. offshore worker identifications below is not provided along with any additional identification requested. Do not submit a request without including one cell phone photo of one of these sets of identification:
    • BOSIET/FOET OPITO-approved certification card issued by a U.S. training facility (a paper certificate instead of a card MUST include the cert number and your U.S. State issued drivers license or U.S. Passport in the same photo as well. Your cert number and name will be verified in the OPITO database.)
    • HUET/THUET OPITO-approved certification card issued by a U.S. training facility (a paper certificate instead of a card MUST include the cert number and your U.S. State issued drivers license or U.S. Passport in the same photo as well. Your cert number and name will be verified in the OPITO database.)
    • MMC BOOKLET issued by the U.S. Coast Guard
    • Commercial Fishing License issued by the U.S. (or a U.S. State) Fish and Wildlife Service (your U.S. State issued drivers license or U.S. Passport must be included in the same photo)
    • Crew member license issued by a U.S. State (your U.S. State issued drivers license or U.S. Passport must be included in the same photo, if the license doesn't not include your photo)
    • 1099 from no later than last or current calendar year issued by a verifiable commercial fishing business as proof of Commercial Fishing Deckhand employment (your U.S. State issued drivers license or U.S. Passport must be included in the same photo)
  • All data is required to get on the waitlist.
  • Only one request per offshore worker.
  • Requests are processed on a first come, first serve basis.
  • If your request is approved and your identification expires while you're on our waitlist, we will email you for an image of your renewed id, as it is required to mail you a PLB.

  • BOSIET or FOET Card with name, face, and expiration date visible:
    • If your card is two sided, take a photo of both the front and the back, then use an app or screenshot to combine them into one image, side by side, to upload here, so we can see your name, face, and certifications listed in one photo.
    • If your cert card does not have an image of your face, you MUST also include a U.S. drivers license or passport in the same photo.

  • Commercial Fishing License with U.S. State Seal or Crewmember License issued by U.S. State:
    • MUST also include your valid U.S. drivers license or passport in the same photo with your name and face visible.
  • Current year or last year 1099 from a commercial fishing business (SSN can be blacked out.)
    • MUST include your U.S. drivers license in the photo with the 1099 for us to validate and approve.
