Consent to Participate in Research Project:
Exploring Telehealth for People with Disabilities in the Pacific Region

We are the Pacific Basin Telehealth Resource Center (PBTRC), 1 of 14 federally funded telehealth resource centers in the United States located at the University of Hawaii. We support improving health care equity and access through the implementation and advancement of telehealth throughout the Pacific Region including American Samoa, Guam, Republic of Palau, Commonwealth of the Northern Mariana Islands, Federated States of Micronesia, Republic of the Marshallese Islands, and Hawaii.

What am I being asked to do?

You are invited to participate in our survey to understand telehealth use for people living with disabilities in the Pacific Region. The survey is multiple choice and expected to take about 10 minutes to complete.

What are the risks and benefits of taking part in this study?
We believe there are potential benefits and little to no risk to you for participating in this survey. Your responses will inform future discussion and direction for PBTRC to better prioritize needed resources for the disability communities in the Pacific and advocate for support based on the highlighted issues. If you become uncomfortable responding to any of the questions, you may skip that question on the survey, or end the survey. Taking part in this study is your choice and participation is voluntary. Non-participation or withdrawal of consent will not result in any penalty or loss of benefits.

* 2. Confidentiality and Privacy
You have the choice to submit an anonymous or confidential survey.

Any information you provide will be kept strictly confidential, and your identity will be protected. Your responses will only be used for the purposes of this study and will not be shared with any third parties without your consent. Once submitted, your survey data will only be viewed by our research team. All the information we collect will remain confidential by ensuring that consent form and survey responses are secured physically and electronically to prevent unauthorized access. Additionally, participant information will be coded so responses will not match with the participant. Only the PBTRC research team will have access to identifying information. At the completion of this research study, all identifying information will be permanently deleted.

At the end of the survey, we added a question to offer an opportunity to participate in a future group discussion about this topic. If you choose not to participate in a future focus group about the topic, you may select "No", and end the survey as an anonymous participant. If you are interested in participating in a future focus group, you may select option "Yes" and provide your name and contact information so we may reach out to you at a future date.

When we report the results of this survey, we will not use your name or any other personally identifying information. If you would like a copy of our final report, please contact the team listed below.

Future Research Studies: The data collected from this study may be used for future research on the topic and we will not seek further approval from you for this purpose.
