Why am I completing this survey?
The Public Health Commander has issued workplace directions where all workers must declare if they are working and/or volunteering at more than one work premises for two or more different employers and must provide details of the other work premises to each employer.
Do I have to complete it? Yes, in adhering to the requirements of the Public Health Commander directions employees must provide a declaration to each employer to advise that the employee is working and/or volunteering at more than one work premises at two or more employers.
What happens to the information?
Each employer must maintain a record of all workers who have disclosed to the employer that they are working and/or volunteering across more than one work premises for two or more different employers. This information will be maintained by the People & Culture team and Western Health treats all personal information provided by an individual confidentiality in accordance with the Privacy and Data Protection Act 2014 (VIC).
