Emergency Medical Services Authority (EMSA), in collaboration with the Coalition for Compassionate Care of California (CCCC), requests your assistance with an important initiative related to the Physician Orders for Life Sustaining Treatment (POLST) form and the upcoming implementation of the California POLST eRegistry (eRegistry). To assess the current knowledge level and identify any areas needing improvement, we are surveying healthcare providers across the state. Aggregate survey results will guide quality improvement efforts to enhance provider education around POLST before the eRegistry launches.

This survey is designed to assess current knowledge of POLST program features and procedures across a wide range of providers and settings. Please answer the questions to the best of your ability, based on your knowledge of POLST and policies and procedures in place in your setting. If you don't know the answer to a given question, you should make your best guess. Your responses are entirely anonymous, and there is no penalty for wrong answers! If you would like to receive a key to the correct answers after the survey deployment is complete, please email CCCC at info@coalitionccc.org to request "POLST survey answers."
